Treatment with Modified Steinmann Pins and Tension Band Wiring Technique in Proximal Humeral Comminuted Fractures with Osteoporosis

Abstract
|
|
|
Purpose: To evaluate the usefulness of internal fixation with m odified Steinm ann pins and tension band wiring technique in com m inuted proxim al hum eral fracture with osteoporosis and the correlations am ong bone m ineral density, age, Neer¡¯s score and period of union.
Materials and methods:: Twelve cases of com m inuted proxim al hum eral fracture with osteoporosis were surgically treated with modified Steinm ann pins and tension band technique, and followed up for an average 20.8 m onths (range, 6¡39 m onths). Average age was 73.6 years old (range, 59¡85 years old). R esults were assessed using Neer¡¯s evaluation criteria.
Results: Excellent results w ere noted in eight cases and satisfactory results in four cases. Radiological union w as obtained in all cases. Tw o cases show ed im pingem ent syndrom e of the proxim al portions of Steinm ann pins, which were m anaged by early rem ovalofthe m etal.O ne case developed operative w ound infection,which were m anaged by antibiotics therapy, irrigation and secondary closure.
Conclusion: Internal fixation using m odified Steinm ann pins and tension band wiring technique for proxim al hum eral fracture, with osteoporosis,m akes com plete union offracture,m inim izes risk ofcom plications,and enables early rehabilitation by rendering relatively rigid fixation.
